SMS Terms & Conditions

These terms govern the EmpowerMyCare SMS notification program. By providing your mobile phone number and opting in to SMS alerts within the EmpowerMyCare app, you agree to these terms.

Program

EmpowerMyCare Care Team Notifications

Message & data rates

Message and data rates may apply. Your mobile carrier may charge you for each SMS you send or receive based on your wireless plan. EmpowerMyCare and Hydromation Corporation do not charge you for SMS messages; however, standard carrier rates still apply.

How to opt in

You opt in to the program when you voluntarily enter your mobile phone number in the EmpowerMyCare app and check the box confirming you want to receive SMS notifications. Opt-in occurs on a web form within the app (https://empowermycare.net) when you add yourself or are added to a care team and provide a phone number.

We do not support opt-in by texting a keyword to a short code.

How to get help — reply HELP

Reply HELP to any message from EmpowerMyCare to receive information about the program and how to contact support. You can also email support@empowermycare.net at any time.

Auto-response to HELP: "EmpowerMyCare: Msgs are care-team notifications. For support email support@empowermycare.net. Reply STOP to cancel. See terms at empowermycare.net/sms-terms.html"

How to opt out — reply STOP

Reply STOP to any message from EmpowerMyCare and you will be immediately unsubscribed from this SMS program. You will receive a final confirmation message, and no further SMS will be sent.

Auto-response to STOP: "You have opted out of EmpowerMyCare SMS notifications. No further messages will be sent. Reply START to re-subscribe."

To re-subscribe after opting out, reply START to the last message you received, or re-enable SMS in Settings → Notifications within the EmpowerMyCare app.

Carrier disclaimer. Mobile carriers (including but not limited to AT&T, T-Mobile, Verizon, US Cellular, and their affiliates) are not liable for delayed or undelivered messages.

Supported carriers

The program supports all major US wireless carriers, including AT&T, T-Mobile, Verizon, and their affiliates. The program is available in the United States only.

Eligibility

You must be at least 18 years old and the account holder or authorized user of the mobile phone number you provide. Do not enter another person's phone number. EmpowerMyCare will not send SMS to a number that has not explicitly opted in through our app.

Privacy

Information you provide for SMS enrollment (your phone number and opt-in preference) is handled under our Privacy Policy. We use Twilio to deliver SMS messages; Twilio processes your phone number solely to route the message to your carrier. We do not share your phone number with advertisers and we do not sell it.

No emergency messages

EmpowerMyCare is an organizational tool and is not intended to deliver emergency or time-critical medical messages. In a medical emergency, call 911.

Changes to this program

We may change these terms or the program at any time. Material changes will be posted here with an updated "Last updated" date. Continued participation after changes means you accept them.
